Membership Application

• For use by applicants who have already completed the membership prerequisites and hope to join the program.

Paper Substitution Form

• For members who have completed an authorized substitute legal research paper in place of the Law & Society course (contact Program Director for topic approval).  Once the authorized paper is completed, have this form signed by the supervising faculty member and submit it along with a copy of your paper.

Internship Completion Form

• For members who have completed a pre-authorized substitute legal internship in place of the Law Forum Seminar course (extremely rare).  If your internship has been pre-approved and finished, please have this form completed by your internship's campus administrator or onsite internship supervisor.

Request for Certificate of Completion

• For members who have completed all of the program requirements and are ready to receive the Law Forum Certificate of Completion.  Whenever possible, please wait until late April to download and complete this form so you can also use it to RSVP for the annual spring ceremony.

Please submit completed forms directly to the Law Forum Director (during office hours) or to Ms. Renee Martin in Social Science Plaza A [SSPA] #3169 during weekday business hours.  Thank you.
